The below Parental Consent for Weekend Training is to be completed and returned each time a cadet attends weekend training.

This Clothing Indent Form is to be completed by cadets requiring an exchange/issue of uniform. Please be sure to give the correct sizes.

The below form, ParQ, is to be completed by the parent/guardian of every cadet who is taking part in obstacle course training.

Form NB16 is to be completed every time the Detachment/Company intends to train outside their usual home location. It is to be delivered to the OC for signing and onwards transmission to County HQ.

Form ACF/100 is to be completed by CFAVs wishing to claim pay and/or home to duty travel expenses. The completed form is to be signed and forwarded to the OC for countersignature and onwards transmission to County HQ. Please send your forms to the OC in good time, so they can be forwarded to County HQ by the month end.

A risk assessment is to be completed for all training that falls outside the Military Safe System of Training (SST). Normal APC training on MOD owned/leased land is covered by the SST.

MOD Form 492 is to be completed every time a cadet or CFAV is injured during ACF training. All CFAVs are to carry at least two paper copies with them at all times, which are to be kept with their copy of Cadet Training Safety Precautions.

The Application for Course form should be completed by CFAVs wishing to bid for a place on an external course, such as those run by the CTT, CTC Frimley Park or CCAT. The completed form is to authorised by the OC/2IC before being forwarded to County HQ.
